Nature of Work:
- Under supervision, assists in assessing health needs of the community, families, and individuals. Assists in planning coordinated nursing services for groups, families, and individuals in the community.
- Consults, teaches, counsels, and refers clients according to diagnosed health needs.
- Performs direct nursing care contributing to prevention and treatment of disease and restoration to the greatest health potential.
- Performs related work as required.
Essential Duties:
- Responsible for supervision of Immunization clinics and vaccine administration.
- Travel to assigned Immunization clinics with co-workers when necessary and assist with the unloading and setting up of equipment.
- Responsible for screening and administering vaccine at immunization clinics as well as computer system operation.
- Receiving vaccines from HHS, proper storage and handling of vaccines Monitor amount of vaccine on hand and ordering if necessary.
- Check temperatures of cooler and refrigerator every AM and PM and maintain logs.
- Handle and dispose of bio-hazard sharp containers.
- Keep supply containers clean and supplied.
- Ensure supplies are ordered as needed, monthly.
- Scheduling dates and times to immunize according to State and CDC guidelines.
- Maintenance of client appointment book, assisting clients with making appointments and updating client records as needed.
- Maintain current provider enrolment list and information.
- Assesses community, family, and individual health behaviours, environmental elements detrimental to health, community resources available for risk reduction, disease prevention, diagnosis, treatment and rehabilitation.
- Applies epidemiological concepts to describe trends and to estimate the magnitude of risk of illness. Describes the health capability based on assessment.
- Forms hypotheses regarding community assessment data to derive inference for nursing services. Teaches groups, peers or individuals’ prevention, including nutrition, safety, infection control, mental health, dental health, physical fitness, human growth and development, hygiene, health resources in the community and interpretation of health regulations and /or medical plan of care. Consults with personnel in day-care centres, schools, worksites, group homes, shelters, elderly housing units, long-term care facilities, other settings to develop effective programs which promote the health of special populations.
- May assist and collaborate with community including peer’s health professionals, community leaders and clients, in the assessment, planning, implementation, and evaluation processes involving health services.
- Assumes responsibility for professional development to achieve mastery of knowledge, values and skills required for community-based nursing care and contributions to the professional growth of others.
- Participates in public health emergency preparedness training and activities.
- Contributes to theory and practice in community health nursing through appropriate research utilization.
Knowledge, Skills, and Abilities Required:
- Theory and processes of the physical, social, biological, and behavioural sciences and their applications to nursing science.
- Concepts and processes related to communities, including epidemiology and biostatics, risk factors and implications for selected populations, resource assessment techniques, environmental factors and social organization.
- Principles and practice of teaching-learning processes, change processes, family dynamics, and cultural diversity.
Qualifications:
- Current licensure in the State of Connecticut as a registered nurse (R.N.) or eligibility to obtain licensure by the date of employment.
- Possession of a State of Connecticut Motor Vehicle License.
