The YMCA Athlone (http://www.ymcaathlone.org), invites applications for a Nurse Coordinator* on an 8-month Fixed Term Contract. YMCA Athlone conducts various Programmes within the Youth and Health Sectors that are conducted at numerous sites in the Klipfontein districts and the greater Cape Town. The incumbent will be responsible for Home-Based Care, Adherence support, and duties related to the health of our clients and the coordination of Community Health Workers.
Responsibilities:
Adhere to the code of professional ethics. Do an assessment on intake or within 48 hours after intake of new clients and develop a nursing care plan if needed. Liaise with local clinics or hospitals for follow-up visits. Liaise with families for follow-up visits. Monitor medication adherence for those on chronic medication. Establish and oversee adherence clubs as agreed by donor SLAs.
Prevent staff from being at risk by implementing regulations regarding correct personal protective equipment/ clothes. Give progress reports on clients of concern at the weekly meeting. Do HCT’s to all patients/clients who have been at risk for HIV. Make sure the TB screening test is done with all patients who are not taking TB treatment or completed their treatment within the last 6 months. Coordinate health awareness campaigns in high-risk communities identified within the areas identified by the Organization, with testing stations when needed. Assist with health education workshops within CHW teams. Submit order form of all stock to the CHC Facility Manager monthly.
Inform the Project Manager: Logistics every Friday of the week of the needs of a driver for the next week. File audits to be done on the last Thursday of a month. Ensure all Covid-19 regulations & protocols are implemented.

Requirements:
Matric (Grade 12), Valid Code 8/10 Driver’s License, Computer skills: Word, Excel and PowerPoint, Degree or Diploma in Nursing Science, Registration with the S.A Nursing Council, Conflict management skills, Communication skills, Report writing skills, Mental health care experience Primary Health care experience (TB/HIV and chronic conditions) (hypertension/diabetes, etc.)
Additional Information:
This position will be based in Cape Town Working hours 40 hours per week, Monday to Friday.
