Description:

The Michael & Mauritia Patcha Foundation and Immigrant Community Services Inc are seeking experienced public health professionals to serve as the HIV/AIDS Navigator in support of the Maryland Department of Health program for targeted outreach to racial and ethnic minority persons with HIV in Montgomery County, Maryland and Prince George’s County, Maryland. The Navigator identifies and facilitates access to HIV care and support services for black clients, particularly African immigrants and African Americans, who are newly diagnosed, sporadically in care or lost to clinical and supportive care services offered within Prince George’s & Montgomery County, MD. The Navigator is a resource and facilitates patients appointments including those for labs, diagnostic areas and follow ups and provides referral leads as needed.

Requirements:

  • Reports to the Program Coordinators
  • Must reside in the DC, Maryland, Virginia metro area.
  • Requires at least a High School Diploma and experience with at-risk populations.
  • Is an outreach, community-based position that normally requires intrapersonal interaction
  • Requires in person meetings with clients. Due to social distancing, PPE will be required to wear at outreach events.
  • Is a part time position offering 10- 20hrs per week.
  • Hours are strictly based on Navigator assignments, meeting or taking some clients to appointments and outreach events. Approximately 3-4 testing outreach events per month (includes some weekends) *Please note this will be an irregular work schedule must be flexible (no set hours).
  • Requires valid and unrestricted authorization to work in the U.S. Visa sponsorship is not available for this position.
  • Demonstrate knowledge and sensitivity to issues surrounding HIV and AIDS and is able to communicate openly on sexual topics and health education without prejudice.
  • Be sensitive to a clientele of diversified cultural backgrounds including LGBTQIA.
  • Possess the ability to speak effectively before groups and actively engage the general public in outreach situations.
  • Be well organized, self-starter with strong verbal, written, computer, and communication skills.
  • Can work independently and as part of a team, be flexible, and manage multiple assignments in a dynamic and fast-paced environment.
  • Have experience working with immigrant and Black Indigenous People of Color (BIPOC) communities.
  • Demonstrate ability to work effectively with a wide variety of people and populations by consistently exercising tact, good judgment, and a proactive, problem-solving focused style.
  • Possess proficiency in the use of computers, including email, internet, virtual meeting platforms such as ZOOM, and office productivity software (e.g. Microsoft Word, Excel, PowerPoint, etc.)
  • Have the ability to work evenings and some weekends.

Roles & Responsibilities:

  • Identify, engage, and follow up with out of care patients.
  • Encourage testing and other diagnostic services for high-risk persons.
  • Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
  • Serve as an essential link between patients and all other care providers.
  • Facilitate appointments for consults and support services within established service standards, including social workers, dietitians, physicians, etc.
  • Document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
  • Be familiar with the HIPAA Privacy Rule and always abide by it.
  • Follow patients throughout the care continuum, including identification and counselling.
  • Work with program management staff and others to collect data, track outcomes, and support strategic planning processes.
  • Provide weekly reports to program coordinators and other groups which demonstrate outcomes and performance improvement activities.
  • Adhere to established productivity benchmarks.
  • Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, social services staff, etc.).

Preferred Qualifications:

  • At least two years’ experience as a Navigator
  • Reside in Prince George/Montgomery County (Maryland)
  • At least two years of HIV/STD related experience or knowledge in HIV/STD partner services, prevention, and surveillance.
  • At least two years of experience in a public health program setting.
  • At least one year of coordination of continuous quality improvement activities and in data-to-care work, (i.e., linkage to care).
