Job Summary:

Journey of Hope Community Housing LLC was formed in 2018 to be a culturally specific, community-based housing solution for youth in Ramsey County. As our Case Manager you will be critical to help us succeed in our mission to provides a safe and encouraging environment for African American adolescents and their families. We offer residential services with supports tailored to the needs of each individual, including assistance with daily activities such as schoolwork, medication administration, and personal cares.

Job Duties:

• Perform social case history on everyone entering the program and write reports as required, leading with intakes and discharges
• Develop case plans for all clients, incorporating person-centered care techniques. Implement case plans, ensuring program staff understand client goals and their role within case plan
• Provide case management, service coordination, supportive counseling and skill building for clients
• Develop systems to track services provided
• Always deliver protection and advocacy of client rights
• Conduct assessments, create, and review care plans, and coordinate services with individuals served and the interdisciplinary team
• Coordinate and oversee individual participation in the community, including special events and memberships
• Provide opportunities for clients to practice and master skillset which is being taught
• Serve as leader and role-model to clients and staff
• Coordinate admissions and discharges in collaboration with the Program Director and the Executive Director
• Routinely communicates with families, directors, day placements, school representatives, medical personnel, and psychologists and provides follow up and documentation in regard to all requests and concerns
• Adhere to professional standards as outlined by protocols, rules, and regulations
• Other duties as assigned by Executive Director and Program Director
