PAH is a holistic short-term residential treatment program (STRTP) for adolescents and non-minor dependents who require specialized mental health services to live safely and successfully at a lower level of care. We serve females, including transgender, gender expansive, and LGBTQ+ youth and non-minor dependents, ages 12 through 18, who are in out of home placements through the Child Welfare or Juvenile Justice systems. Our youth are living with a range of complex behavioral health needs, including those related to complex trauma and/or adverse childhood experiences, identity formation issues, depression, anxiety, and self-concept and family issues that inhibit well-being and healthy relationships. We strive to create an interactive and enriching environment that addresses the developmental, physical, emotional, social, mental health and spiritual needs of each youth served by teaching basic life and independent living skills, promoting safety and stability, and preparing for transition to adulthood.

Position Summary:

The Care Coordinator provides case management and mental health services to youth ages 12-18 years old placed at the STRTP. The Care Coordinator Case works with the youth to access needed vocational, rehabilitative, or other needed community services. The Care Coordinator is part of a multidisciplinary team that engages and collaborates with the youth, their family, Child Welfare, and other members of their Child and Family team to support the youth to successfully transition to a lower level of care. The Care Coordinator ensures that the activities and services of all parties involved in the team are coordinated and comprehensive. The Care Coordinator takes a strengths-based, trauma-informed, culturally relevant, and client-centered approach to working with youth. The Care Coordinator may be required to provide services on the evenings and weekends.

Essential Duties and Responsibilities:

  • Partners with the youth and team to ensure that assessment, service planning, and coordination of service, including urgent services, includes the youth’s goals and preferences for services and mental health services appropriate to meet the needs of the youth
  • Participates in mental health assessment activities, including gathering relevant documents and information from Child Welfare and the team
  • Participates in Child and Family Team meetings and facilitates CFT meetings as needed
  • Partner with youth in promoting the involvement of family and other informal and formal supports.
  • Provide collateral services to youth’s significant support people
  • Coordinates efforts towards youth and family access to natural, professional, and community supports
  • Assists with identifying and pursuing resources, ensuring access to needed services and monitoring the progress of the youth
  • Placement services, including providing supportive assistance to the youth in the assessment, determination of need, and securing of adequate and appropriate living arrangements
  • Rehabilitation services to assist in restoring or maintaining a youth’s functional, daily living, and social skills
  • Provides intensive home-based services to youth and families as needed
  • Partner with youth in identifying and accessing community resources
  • Partner with youth in identifying obstacles to meeting educational, vocational, social, and independent living goals. Develop strategies to overcome these barriers.
  • Completes documentation in line with all applicable agency, county, state, and regulatory requirements, and timelines
  • Participates in regular team meetings
  • Participates in crisis services and emergency intervention protocols as needed
  • Develops interagency and community network relationships which improve access to relevant community services and benefits.
  • Other duties as assigned to ensure the safety and well-being of each youth.

Qualifications:

  • Bachelor’s degree in social work, psychology, counseling, or related social services or behavioral health field from an accredited university or college
  • At least 21 years of age
  • At least one year’s experience providing services to adolescents, preferably current and/or former foster youth and/or youth involved in the juvenile justice system and their families
  • California Department of Justice and FBI fingerprint clearance.
  • Must have a dependable personal vehicle, valid California driver’s license, and proof of insurance.

Knowledge, Skills, and Abilities:

  • Competence in positive client engagement and communication
  • Familiarity with principles, models, and techniques for serving impoverished and violence-impacted communities
  • Ability to establish and maintain effective working relationships with a variety of clients and staff from diverse cultural backgrounds
  • Commitment to working in a diverse workplace, and self-awareness regarding issues of race, ethnicity, class, sexual orientation, gender identity, religion/spirituality, and disability.
  • Cultural competence skills for working with African American, Latino, LGBTQ+, and disenfranchised populations is necessary
  • Ability to establish and maintain effective working relationships with a range of stakeholders, families, schools, juvenile justice and child welfare systems personnel, partner agencies, and community members.
  • Ability to partner with youth to implement person-centered, needs-driven, strengths-based, and culturally relevant services
  • Ability to maintain assigned caseload and achieve productivity expectations
  • Highly organized, with ability to prioritize and follow-through with various tasks and treatment activities
  • Ability to view families, including biological, resource, and chosen families, as resources and support systems for youth, and commitment to involve families in services
